CLDS Service Coordination Pilot Project - Service Providers

On April 6th, 2021 Community Living disABILITY Services (CLDS) partnered with three CLDS-funded service providers – Life’s Journey, New Directions and St.Amant – to establish the Service Coordination Pilot Project.  

  • The purpose of this project was to develop an additional service delivery model for agencies that have the capacity and infrastructure to assume the case management function for the participants they support.  As such, these three service providers were selected for the pilot based on the case management services they already deliver to CLDS participants.   
  • This project aimed to streamline processes, clearly define roles and responsibilities between government and service delivery agencies, and improve service delivery to participants receiving residential services.

In order to evaluate the outcomes of the Service Coordination Pilot Project, staff from both CLDS and the three pilot agencies are being asked to respond to pre and post pilot surveys with questions on a variety of topics linked to the key outcomes this project aims to achieve.  

These survey questions were first administered to you in the early days of the project in order to develop a baseline of staff perspectives about service coordination before the pilot project began.  They are being administered again now to determine staff perspectives about service coordination, one year after the project was implemented. 

A comparison of staff responses regarding service coordination before and after the pilot project will assist CLDS in evaluating whether the project achieved intended goals.
