CLDS Service Coordination Pilot Project - Families and Support Networks

Community Living disABILITY Services (CLDS) has collaborated with St.Amant, New Directions and Life’s Journey to establish the Service Coordination Pilot Project, which launched in April 2021. 

The purpose of this project was to create another service delivery model for agencies to take over the case management of the participants they support. These three agencies were chosen for the pilot project based on the case management services they already deliver to CLDS participants. 

The project aims to streamline processes, clearly define roles and responsibilities between government and funded agencies, and improve service delivery to participants receiving residential services.

In order to evaluate the outcomes of the pilot project, the CLDS program has developed a survey to gather input from families and members of the support networks of participants who are involved in the pilot project.  

You are being asked to complete this survey because you have been identified as a family member, friend, advocate or support person to a CLDS participant who is receiving services from one of the pilot agencies. These surveys are anonymous and responses are kept confidential. Please do not include any identifying information about yourself or the CLDS participant.

As people who care for and provide support to CLDS participants involved in the pilot project, your opinion is very important. Thank you for taking the time to share your feedback and opinions, as we strive for positive collaboration with the community and the continued delivery of quality services.
